To use this option, you must first obtain a password by calling the Family Wealth Alliance at 630-684-2210, then click here. Our survey host system allows you to fill out a portion of your questionnaire, then return subsequently as many times as needed and use your password again to sign on and complete it. When you finish the final screen, your survey is completed and will be automatically submitted.

2. Download the questionnaire in editable Adobe Acrobat PDF format
You can complete the questionnaire and email it back by clicking the send button on the final page. Or you can print out the completed questionnaire and fax it back to us at 866-410-5532.
